✱ Overview

What is SS-31?

SS-31, also known as elamipretide, is a small synthetic peptide designed to concentrate in mitochondria. It binds cardiolipin, a lipid in the inner mitochondrial membrane that helps organize the machinery of energy production.

It has been studied in several clinical programs, including primary mitochondrial myopathy, Barth syndrome and heart failure, with mixed results across indications.

✱ Mechanism

How it works

Cardiolipin anchors and stabilizes the protein complexes of the electron transport chain. When cardiolipin is damaged by oxidative stress, energy production becomes less efficient and more reactive oxygen species are generated.

By associating with cardiolipin, SS-31 is proposed to stabilize membrane structure, improve electron transport efficiency and reduce oxidative stress at its source.

01

Mitochondrial disease (Barth syndrome)

Established clinical evidence

Clinical data supported FDA accelerated approval of a branded elamipretide product for Barth syndrome in 2025. Continued approval may depend on confirmatory evidence.

02

Mitochondrial membranes

Preliminary findings

Laboratory work consistently shows SS-31 binding cardiolipin and stabilizing inner-membrane structure.

03

Oxidative stress

Preliminary findings

Cell and animal studies show reduced mitochondrial reactive oxygen species.

04

Muscle function

Preliminary findings

A phase 3 trial in primary mitochondrial myopathy did not meet its primary endpoint.

05

Investigational applications

Experimental hypothesis

Heart failure, kidney and age-related muscle research remain investigational.

✱ FAQ

SS-31 questions

A specific branded elamipretide product was granted accelerated approval for Barth syndrome. The research vial listed here is not that product and is not approved.

No. Because this listing is not an approved treatment, quote requests are disabled. You can submit a general research-information inquiry.

It binds cardiolipin in the inner mitochondrial membrane, which researchers believe helps stabilize energy production.
